How do I request a Home Visit?

Please do not ask a doctor to visit you at home unless the patient is really too ill or infirm to come to the surgery.  Remember the Doctor can see five patients in surgery in the time it takes to make one home visit and there are better facilities for examinations and treatment in the surgery.

Except in the case of acute emergencies all home visits should be requested before 10.30am, please explain what the problem is to the receptionist as this will help the doctor.  Home visits will be made after the end of surgery.

How to I obtain a Repeat Prescriptions?
If you are on repeat prescriptions, this is how to order them:
  • Place a tick next to the items you require on your prescription order form (the right hand side of your prescription)
  • Drop the form in to us via the outside post box on the gate or in the prescriptiom box inside the medical centre.
  • Use our online prescription request service (available towards the end of 2009) 
  • By post , using the prescription order form.

Telephone requests will NOT be accepted.  Please do not telephone the Out of Hours Service to request a prescription.  They cannot help you.

If you supply a stamped addressed envelope we can post the prescription back to you or your prescriptions can be collected by the local pharmacies Co-operative (Richmond Road), Lloyds Chemist (Handsworth), Rowlands (Woodhouse), Associated Chemist Wicker or Boots.

Please allow 2 working days for your prescription to be completed

If your prescription is due for review you need to allow 3 working days

 

How do I get telephone advice?

The duty doctor will be happy to give you advice on the telephone when he or she is not consulting. The receptionists have been asked not to interrupt the doctor during consultations, unless your call is urgent.

If you would like to speak to the Doctor/Nurse please inform the receptionist that you would like the Doctor/Nurse to return your call, the receptionist will ask you for details of the problem and will request a telephone number for you to be called back on.

The Doctor/Nurse will try on two occassions to make contact.  If for whatever reason you do not answer but have a messaging system they will leave a message to say they have telephoned. 

When can I use the Out of Hours Service?

Please only request an out of hours consultation if you think the patient really cannot wait until we are next open.  If you need to see someone out of hours telephone 0845 122 2831 and you will be transferred to the out of hours provider.  Remember this is an out of hours emergency service and should not be used for routine medical services.  If you need advice please contact NHS Direct on 0845 4647 or www.nhsdirect.nhs.uk.  There is also the Sheffield Walk-In Centre located on B Floor of the Royal Hallamshire Hospital, this service is open from 8.00am to 10.00pm seven days per week.  Consultations with community pharmacists are also available at your local pharmacy.

How do I obtain access to my patient information?

All staff at the practice are bound to maintain patient confidentiality.  Any proven breach of confidentiality will be treated extremely seriously.  Confidentiality also extends to family members therefore, information about you will not be divulged to a family member without your consent.
